Victoria Village is looking for an Admission Coordinator to join our Village team
Reporting to the Director of Resident & Family Services, the Admission Coordinator is responsible for the admissions and tour in accordance with Victoria Village Manor policies. The Admission Coordinator will ensure effective collaboration with all other members of the interdisciplinary team; including but not limited to nursing, dietary and social worker to coordinate and process all admissions.
The Admission Coordinator is required to perform their duties in a manner consistent with the Victoria Village Mission and core values, standards of employee conduct, staff code of ethics, employee guiding principles, ensure resident safety, and demonstrate customer service excellence.
Duties and Responsibilities:
**Admission/Discharges/Transfers**
- Liaises with Home and Community Care Support Services regarding our 128 bed LTC facility
- Tracks all vacancies to ensure beds are filled in a timely manner
- Maintains occupancy at 98% or better
- Sends acceptance or rejections letters to residents / SDM’s within 5 days
- Maintains community and internal waitlist
- Monitors internal waitlist and makes appropriate internal transfers
- Notifies departments of Admissions/Discharges/Transfers
- Arranges and facilitates admission meetings with new families/residents and the interdisciplinary team, and completes MDS RAI face sheet
- Prepares and completes paperwork for Admissions and completes with families/SDM’s/Residents (consents, advanced care directives, unfunded services, etc.)
- Prepares room (ensures room is ready, name plate, personal care items)
- Discharges: connect with families if room hasn’t been cleared out; notifies
- Monitors all new Admissions during transition into LTC
- Provides emotional support to residents and families
- Assessments and interventions; Completes all new admission initial assessments (RAI MDS Face Sheet),
- Assist in the coordination of Care Conferences on an initial and annual basis for residents and their loved ones.
**Tours**
- Arranges and conducts facility tours
- Answers inquiry calls
OTHER DUTIES AS ASSIGNED
**Ministry of Health Compliance**
- Fosters good working relationships with relevant persons within the Ministry of Health and LTC;
- Knows and interprets MOHLTC legislation, regulation and process and Victoria Village Manor standards; Implements processes and systems to ensure compliance to all department relevant standards on an ongoing basis;
- Ensures sustainability of all systems and processes in place to achieve compliance.
- Participates in drills to prepare for emergencies and attends all education sessions in relation to emergency preparedness.
**Qualifications**:
- possess a minimum of a Bachelors degree in Health Sciences or equivalent from a recognized University
- Must have experience in working as part of an interdisciplinary team;
- Must possess excellent leadership and communications skills;
- Must possess a sound knowledge of gerontology;
- Knowledge of Substitue Decision Maker’s Act as well as Capacity Assessments preferred

Pay: $25.50-$28.05 per hour
