Resident Service Coordinator

The Opportunity:
The Resident Service Coordinator is responsible to the Director of Care and is responsible for coordinating resident services within the Retirement and Supportive Living Community. The resident service coordinator ensures the implementation and coordination of resident care and services in accordance with current applicable provincial standards, guideline and regulations to ensure that the highest standard of care and services are delivered and maintained at all times.
Within the scope of this position, the Resident Service Coordinator creates a culture that brings out the best in all who live and work in our communities and supports residents to enhance their well-being by:
- Promoting care partnerships
- Embracing a whole-person approach to care and service delivery
- Respecting individuality and maximizing independence
- Supporting and being responsive to changing resident needs
- Celebrating the life journey with meaning and purpose
Key Responsibilities:
- Ensures Residents are welcomed into the community in accordance with AgeCare policies and provincial standards
- Provides support to all care staff to attain and maintain quality care within a person-centered model of care
- Support leadership to assist with ensuring and sustaining an atmosphere of safe, quality resident centered care
- Monitor and coordinate the care team to achieve quality resident and family centered care
- Support a culture of safety in the workplace for residents, visitors and employees
Qualifications:
- Health Care Aide certification and registration in the Health Care Aide Directory required
- Must be fluent in English, both orally and in writing
- At least 3 years’ experience as an HCA in continuing care setting, demonstration of leadership abilities with strengths in supervision, leadership, judgement and interpersonal skills
- Education or certification in leadership and conflict management an asset
- 5+ years’ experience working with AHS Medication Assistance Program (MAP) considered an asset
