Salary and BenefitsThe estimated salary range for this position is CAD $41.42 - $59.52 per hour, depending on experience and qualifications.
We offer a comprehensive compensation package, including four weeks of vacation, health benefits, and a pension plan.
Job DescriptionAs a Patient Care Coordinator, you will play a vital role in providing direct patient care by assessing, planning, and implementing nursing interventions to improve patient outcomes.
You will collaborate with patients, families, and healthcare teams to develop and implement individualized care plans, promote person-centered care, and ensure seamless transitions throughout the care continuum.
This is an excellent opportunity to grow your skills and knowledge in a dynamic healthcare environment, working collaboratively with a multidisciplinary team to deliver high-quality patient care.
Required Skills and Qualifications- A degree in Nursing from an accredited institution and current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Current certification in Basic Cardiac Life Support (BCLS) is required.
- Excellent communication, interpersonal, and problem-solving skills are essential for success in this role.
- Able to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
