Patient Care Coordinator

We are seeking a compassionate Registered Practical Nurse (RPN) to provide high-quality nursing care to patients in various settings. The ideal candidate will have a valid RPN license in Canada and completion of a recognized Practical Nursing program.
Key Responsibilities:- Assess patients' needs and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report any changes in condition.
- Collaborate with healthcare professionals to ensure coordinated care.
- Provide education and support to patients and their families.
- Valid RPN license in Canada.
- Completion of a recognized Practical Nursing program.
- Previous experience in a healthcare setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment.
