Job Summary:

The Crisis Specialist will work in an inter-agency, multi-disciplinary team to provide crisis support services to seniors in their homes or community locations. The successful candidate will have a strong background in crisis support, geriatric mental health, and case management.

Key Responsibilities:
  • Participate in crisis shifts and provide crisis support services to seniors in their homes or community locations.
  • Perform psychosocial and geriatric assessments to identify the needs of seniors and develop a care plan.
  • Provide short-term intensive case management services to a caseload of seniors with mental health and substance use issues.
  • Collaborate with clients, family members, and caregivers to develop and implement a care plan.
  • Provide supportive counseling and advocacy services to seniors and their families.
  • Work closely with community professionals and agencies to ensure a continuum of care for seniors.
  • Participate in ongoing training and education to stay up-to-date on best practices in crisis support and geriatric mental health.
Requirements:
  • Masters of Social Work or post-graduate degree in a related field with 2+ years of experience in crisis support and geriatric mental health.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Strong assessment, clinical, and intervention skills, with a client-centered approach.
  • Effective communication, documentation, and interpersonal skills.
  • Ability to work in a multi-disciplinary team and collaborate with community professionals.
