Social Services Professional

Mon Sheong Foundation is seeking a highly skilled Social Services Professional to join our team at Mon Sheong Dr. Paul Woo Long Term Care Centre.

Key Responsibilities:

  • Engage with residents and their families to provide counseling and case management services.
  • Conduct psychosocial assessments, coordinate admissions and discharges, and organize support groups and educational programs.
  • Oversight of the Resident Council and Family Council, and collaboration with the Activation Supervisor to develop and support volunteer and pastoral services.
  • Work with Ontario Health Home to establish and manage the waiting list, and coordinate the admission of residents to fill beds in accordance with the Pre-Occupancy Plan.
  • Prepare statistical reports and other reports relating to Social Services, and complete social, emotional assessments, and apply tests for cognition of all residents.

Requirements:

  • Degree in social work, preferably MSW, and current membership of Ontario College of Social Workers and Social Service Workers (OCSWSSW) in good standing.
  • Knowledge of gerontology and experience in Long-Term Care settings preferred.
  • Excellent interpersonal communication, organizational skills, and human skills.
  • Demonstrated family, group, and crisis intervention counseling skills.
  • Strong team work, consultation skills, and ability to work as a member in an interdisciplinary team.
  • Proficiency in both English and Chinese.
