The Role

This Community Health Care Specialist position offers a unique opportunity to work in a fast-paced community nursing environment. As an RPN, you will have scheduled visits with patients in various settings, working closely with RNs for more complex cases. You will provide care to patients of all ages, addressing their individual needs and helping them remain comfortable in their homes.

We offer a competitive salary, ranging from $93,500 to $107,000 per year, based on experience and location.

Your Responsibilities

  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Provide safe and ethical nursing care, utilizing assessment tools and techniques as needed
  • Collaborate with patients, families, and other healthcare professionals to develop and implement care plans
  • Maintain accurate and up-to-date patient records

Requirements

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in delivering community care (an asset)
  • Reliability and flexibility
  • Willingness to travel to and from patient residences
