Job Summary
We are seeking a dedicated Registered Practical Nurse (RPN) to join our team at SE Health. As a key member of our care team, you will provide high-quality patient care and support to clients in our Leamington Clinic and the surrounding community.
Key Responsibilities
- Complete comprehensive client assessments and develop personalized care plans
- Implement and evaluate care based on best evidence and practices
- Support clients through care transitions by collaborating with community support services
- Work collaboratively with other care team members and support functions

Requirements
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice and excellence in care
- A valid driver's license and vehicle (travel required)
- Availability to work days, evenings, nights, and every other weekend
