We are seeking a highly skilled and compassionate Nurse Clinician Case Manager to join our Neuromotor Program team at BC Children's Hospital and Sunny Hill Health Centre in Vancouver, BC.
Job Summary:The successful candidate will be responsible for providing high-quality patient care, coordinating assessments, and managing referrals for children with neuromotor conditions. This is a unique opportunity to work with a multidisciplinary team and make a meaningful impact on the lives of our patients and their families.
Key Responsibilities:- Initiate and coordinate assessments, respond to patient, family, and healthcare member inquiries, and provide anticipatory guidance and case management.
- Direct the intake/access process for the NMP team, performing duties such as receiving, collecting, and documenting child-specific information, conducting interviews, and problem-solving with families and other team members.
- Collaborate with other healthcare team members to evaluate family-centered patient management, reviewing patient/family history, developing individualized service plans, and meeting with other team members to discuss and review recommendations.
- Coordinate teaching of patients/families with a new neuromotor condition and/or sensory impairment diagnosis, developing teaching resources, referring to and liaising with other healthcare professionals, and acting as a resource for other healthcare professionals.
- Provide supportive case management to families, ensuring they receive assistance with patient advocacy, information regarding program, team, or community health and educational resources, and referrals to appropriate community services and/or agencies.
- Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM).
- Three years recent related and progressively responsible experience, including previous experience working with the population and/or in a child development and rehabilitation setting, or an equivalent combination of education, training, and experience.
- CPR Training – Basic Cardiac Life Support (BCLS) – Level I.
- Valid BC Driver's License and access to vehicle and ability to travel.
