Nurse Clinician Case Manager, Neuromotor Program

Key Responsibilities

  • Develop and implement comprehensive care plans for patients with neuromotor conditions, collaborating with multidisciplinary teams to ensure seamless care coordination.
  • Conduct thorough assessments, identify patient needs, and provide anticipatory guidance to families, promoting optimal health outcomes.
  • Coordinate clinic bookings, manage waitlists, and liaise with diagnostic and health services to facilitate patient referrals.
  • Direct the intake process, receiving and documenting child-specific information, conducting interviews, and problem-solving with families and team members.
  • Collaborate with healthcare teams to evaluate family-centered patient management, reviewing patient history, developing individualized service plans, and meeting with team members to discuss recommendations.
  • Provide supportive case management to families, ensuring they receive assistance with patient advocacy, program information, and referrals to community services and agencies.

Requirements

  • Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM).
  • Three years recent related and progressively responsible experience, including previous experience working with the population and/or in a child development and rehabilitation setting.
  • CPR Training – Basic Cardiac Life Support (BCLS) – Level I.
  • Valid BC Driver's License and access to vehicle and ability to travel.
