Nurse Clinician Case Manager, Neuromotor Program

Key Responsibilities

  • Develop and implement comprehensive case management plans for patients with neuromotor conditions, ensuring seamless coordination with healthcare teams and community resources.
  • Provide expert nursing care and guidance to patients and families, addressing their unique needs and concerns.
  • Collaborate with multidisciplinary teams to evaluate patient-centered care and develop individualized service plans.
  • Design and deliver educational programs for patients and families, promoting empowerment and self-management.
  • Facilitate referrals to community services and resources, ensuring timely access to care.

Requirements

  • Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM).
  • Three years recent related and progressively responsible experience, including previous experience working with the population and/or in a child development and rehabilitation setting.
  • CPR Training – Basic Cardiac Life Support (BCLS) – Level I.
  • Valid BC Driver's License and access to vehicle and ability to travel.
