Job Title: Nurse Clinician Case Manager, Neuromotor Program

We are seeking a highly skilled and compassionate Nurse Clinician Case Manager to join our Neuromotor Program team at BC Children's Hospital and Sunny Hill Health Centre in Vancouver, BC.

Job Summary:

The successful candidate will be responsible for providing high-quality patient care, coordinating assessments, and managing referrals for children and youth with neuromotor conditions. This is a unique opportunity to work with a multidisciplinary team and make a meaningful impact on the lives of our patients and their families.

Key Responsibilities:
  • Initiate and coordinate assessments, respond to patient and family inquiries, and provide anticipatory guidance and case management.
  • Direct the intake/access process for the Neuromotor Program team, including receiving and documenting child-specific information, conducting interviews, and assessing referrals.
  • Collaborate with the healthcare team to evaluate family-centered patient management and develop individualized service plans.
  • Coordinate teaching and education for patients and families with new neuromotor conditions or sensory impairments.
  • Provide supportive case management to families, ensuring they receive assistance with patient advocacy, information, and referrals to community services.
Requirements:
  • Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses & Midwives (BCCNM).
  • Three years recent related and progressively responsible experience, including previous experience working with the population and/or in a child development and rehabilitation setting.
  • CPR Training – Basic Cardiac Life Support (BCLS) – Level I.
  • Valid BC Driver's License and access to vehicle and ability to travel.
Preferred Qualifications:
  • Knowledge of community-based services for children with neuromotor/developmental conditions.
  • Excellent communication skills, both oral and written.
  • Strong leadership and organizational skills.
  • Ability to relate effectively to children and their families, dealing effectively with those experiencing stress and/or in crisis.
  • Ability to collaborate and work effectively as a member of an interdisciplinary care team and function as a nursing resource.
  • Ability to plan, deliver, and evaluate educational sessions based on principles of adult education.
  • Commitment to developing knowledge and understanding of legislative obligations and provincial commitments found in foundational documents, including Truth & Reconciliation Commission's Calls to Action (2015), In Plain Sight (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP), Reclaiming Power and Place Missing and Murdered Indigenous Women & Girls Calls for Justice (2019), the Declaration Act Action Plan, and Remembering Keegan: A First Nations Case Study.
