Indigenous Health Advocate

At Bluewater Health, we are seeking a skilled Indigenous Health Advocate to join our team. This is a full-time position with an estimated annual salary of $60,000-$80,000, depending on experience.
About the RoleThe successful candidate will work in compliance with all relevant legislation and hospital policies, practicing cultural safety and excellence in patient care. They will have effective communication and interpersonal skills, be able to work independently and as part of a team, and possess strong attention to detail and accuracy.
Key Responsibilities- Engage with patients early through identification, care team referrals, or community connections.
- Provide support, information, and advocacy for patients attending the Emergency Department.
- In consultation with patients, families, and healthcare teams, individualize care paths to reflect interventions meeting identified needs and goals, while promoting cultural appropriateness, safety, privacy, and confidentiality.
- Develop patient-centered plans of care incorporating traditional healing, culturally safe practices, community connections, and preventative health services based on client choice.
- Member in good standing with a current Certificate of Registration with a Regulated Health Professions College in Ontario (preferred).
- Nursing or social work/social service background (preferred), with other allied health professionals considered.
- Certificate or diploma in Addictions (preferred).
- Identifies as Indigenous (First Nation, Metis, Inuit) with lived experience.
- Indigenous-based Trauma Informed education (preferred).
- Experience working with First Nation communities (desired).
- Previous experience working in Indigenous health; established relationships with Indigenous community health services, local hospitals, regional/local health centers, and community-based health services.
- Knowledge of case management/system navigation and community resources, particularly Indigenous specific.
- Excellent understanding of Indigenous communities, regional/local health centers, and community-based health services.
- Experience with and knowledge of local/regional Indigenous language, cultures, customs, traditions.
- Demonstrated ability to work with Indigenous Elders, Traditional Healers, and Knowledge Keepers.
