About the Role

We are seeking an experienced LPN to join our team as a Care Coordinator for Mental Health and Addiction Services. In this role, you will be responsible for assessing clients' needs, developing individualized treatment plans, and implementing interventions to support their recovery. Your expertise and compassion will make a significant difference in the lives of those we serve.

Key Responsibilities
  • Conduct comprehensive assessments of clients' mental health and addiction issues, identifying strengths and areas for improvement.
  • Develop and implement individualized treatment plans that address clients' unique needs and goals.
  • Provide education and support to clients and their families on mental health and addiction recovery strategies.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Document client progress and adjust treatment plans as needed.
Requirements
  • Current LPN license in good standing.
  • Experience working in mental health and addiction services is essential.
  • Knowledge of trauma-informed care practices and 12-step principles is an asset.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and collaboratively as part of a dynamic team.
