Job Description:

We are seeking a highly motivated and compassionate Mental Health Support Specialist to join our team at the Provincial Assessment Centre. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care and support to individuals with complex mental health needs.

Key Responsibilities:
  • Monitor clients in accordance with treatment objectives, observing behaviors and symptoms, and documenting observations in accordance with standard procedures. Report medical, behavioral, and other problems as required.
  • Participate in the admission, case planning, and discharge process for clients by conducting interviews, gathering history, collecting relevant records, attending client case meetings, and assisting with individual care plan development.
  • Establish rapport with clients based on respect, provide emotional support, assist with problem-solving, and recommend changes to treatment goals with the interdisciplinary team.
  • Assist clients with daily living activities, escort them to appointments, and act as a role model to facilitate social behaviors.
  • Coordinate services with medical professionals and community resources, act as an advocate for clients, and provide information on community services.
Requirements:
  • Diploma from a recognized Social Services Support Worker educational program or two years' recent experience in a related field.
  • Valid driver's license for local area travel requiring the use of a personal vehicle.
