Qualicare Burlington is seeking a skilled and compassionate Registered Nurse to join our Community Visiting Nursing Program. As a member of our team, you will provide high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Provide comprehensive nursing care to patients in their homes, including wound care, IV management, and diabetic care.
- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient progress and maintain accurate records.
- Participate in ongoing education and professional development to stay up-to-date on best practices and industry advancements.
- Current registration with the College of Nurses of Ontario (CNO) and in good standing.
- Experience in Home and Community Care is an asset.
- Current BCLS certification (Level C).
- Demonstrated clinical assessment and patient/family teaching skills.
- Demonstrated effective communication skills.
- Charting on Procura is an asset.
