Job Summary

The Nurse Practitioner in the Community & Population Health Program will be responsible for providing high-quality patient care to individuals with complex health needs. This role will involve working within an interdisciplinary team to develop and implement care plans that promote seamless transitions from hospital to community.

Key Responsibilities
  • Assess and treat patients with multi-morbidity, often impacted by social determinants of health and frailty
  • Develop integrated care plans to support patient transitions and post-discharge follow-up
  • Provide direct patient care, including assessment, treatment, and education
  • Collaborate with healthcare teams to ensure comprehensive care and support
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery
Requirements
  • Current Certificate of Registration in the College of Nurses of Ontario Extended Class - Adult
  • Completion of a Masters Degree in Nursing
  • Minimum of 5 years recent (within 2 years) clinical experience working with older adults presenting to hospital with multi-morbidity, frailty, and social care needs
  • Demonstrated knowledge of the chronic disease management framework
  • Demonstrated knowledge of community resources to support integrated discharge planning with community partners
  • Demonstrated ability to critically appraise the literature, evaluate, and implement best practice protocols
  • Demonstrated knowledge in quality improvement and the use of quality improvement methods and tools
  • Proven leadership abilities, including the ability to think strategically, influence decision making, affect culture and behavior
  • Proven effective communication and presentation skills
  • Proven success in implementing innovative, effective, value-added initiatives using change management and project management theory and frameworks
  • Demonstrated ability for independent action, initiative, problem solving, organization, and prioritization
  • Demonstrated critical thinking, interpersonal, and communication skills, both written and verbal
  • Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations
  • Demonstrated ability to utilize information technology
