We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Good Shepherd Centres. As a key member of our Primary Care Access and Attachment Team (PCAAT), you will play a vital role in providing comprehensive primary healthcare services to vulnerable populations in our community.
Key Responsibilities- Provide primary care to individuals served by PCAAT, including health promotion, disease prevention, treatment of injury and illness, consultation, and support.
- Work collaboratively with interdisciplinary professionals to connect with vulnerable populations and increase access and attachment to Primary Care.
- Develop and implement health promotion strategies to support individuals through all aspects of their health and illness.
- Utilize a holistic approach and a client-centered approach to care, paying special attention to psychological, social, cultural, and environmental factors.
- Communicate and collaborate with hospitals and other community health services to ensure seamless care coordination.
- Bachelor Degree in Nursing and Registration in the Extended Class with the College of Nurses of Ontario.
- Two years' experience as a Nurse Practitioner, RN, (EC) preferably in a community setting.
- Experience working with mental health, addiction, and concurrent disorders.
- Experience and sensitivity in working with culturally diverse populations, including individuals newly arrived in Canada.
- Committed to intersectional anti-racist/anti-oppressive practice, harm reduction, and trauma-informed care.
