We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Good Shepherd Centres. As a key member of our Primary Care Access and Attachment Team (PCAAT), you will play a vital role in providing comprehensive primary healthcare services to vulnerable populations in our community.
Key Responsibilities- Provide primary care to individuals served by PCAAT, working within the full scope of the RN (EC) practice.
- Act as an important communication link between PCAAT staff and service users and providers to ensure effective communication and timely sharing of information.
- Coordinate and schedule service delivery between various program sites, clinic sites, and on-site physician services.
- Provide comprehensive assessments, health examinations, and interpret diagnostic, screening, and lab tests and reports.
- Formulate diagnoses and communicate and interpret these to clients and family members.
- Provide care to well individuals of all ages and their families, emphasizing health enhancement and risk reduction.
- Treat illnesses within the scope of practice through pharmacological and complimentary therapies.
- Provide education, information, and support to individuals to promote health and wellness.
- Utilize a holistic approach and client-centered care, paying attention to psychological, social, cultural, and environmental factors.
- Triage, communicate, and collaborate with hospitals and other community health services.
- Assist in linking clients with community primary care physicians as necessary.
- Respectful and compassionate demeanor.
- Bachelor Degree in Nursing and Registration in the Extended Class with the College of Nurses of Ontario.
- Two years' experience as a Nurse Practitioner, RN, (EC) preferably in a community setting.
- Experience working with mental health, addiction, and concurrent disorders.
- Experience and sensitivity in working with culturally diverse populations, including individuals newly arrived in Canada.
- CPR/First Aid certification and WHMIS training and/or willingness to certify.
- Committed to intersectional anti-racist/anti-oppressive practice, harm reduction, and trauma-informed care.
- Ability to work both independently and as a member of a team.
- Outgoing and adaptable to working in a fast-paced and changing environment.
- Creativity and flexibility in achieving desired outcomes.
- Hospitable in dealings with clients and other key stakeholders.
- Available, respectful, and responsive to the needs of others.
- Strong written and verbal communication skills.
- Good interpersonal and organizational skills.
- Ability to work with various record-keeping technologies, including Electronic Medical Record programs.
