About the Role

The Licensed Practical Nurse (LPN) is a key member of our care team at Retirement Concepts, responsible for coordinating care delivery and providing individualized resident-centered care. This role reports to the Director of Care and is a role model and team leader to an assigned unit.

Responsibilities:

  • Assess residents' needs and implement and evaluate individualized care plans.
  • Ensure residents' rights are observed and adhere to a holistic philosophy of wellness.
  • Liaise with physicians, community agency representatives, families, visitors, volunteers, other disciplines, and departments to ensure quality resident-centered care is delivered.
  • Assess and treat all wounds on the unit and act as a resource to care staff for strategies to prevent skin breakdown.
  • Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
  • Monitor and report changes in physical and psychological conditions of residents. Document changes on the resident health record, notify family and physician of changes in condition.
  • Active participant in Interdisciplinary Care Conferences.
  • Participate in care plan reviews to ensure current and individualized care plans.
  • Support the administrative requirements at the community in accordance with established standards, including but not limited to; admissions, discharge, and ensuring confidentiality is maintained at all times.
  • Actively participate in orientation of new staff, community programs, meetings, committees, and educational in-services.
  • Provide coaching and direction to care staff as needed and use in-the-moment education to improve resident care, which includes completing all Point of Care (POC) documentation. Seek assistance/support from RN or designate as needed.
  • Maintain an up-to-date knowledge of community policies and procedures, particularly those concerning emergency procedures, including equipment management, guidance, and assistance on safe work practices.
  • Perform other related duties as required.

Qualifications and Skills:

  • Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long-term care facility an asset.
  • All successful applicants must pass the vulnerable sector Criminal Record Check applicable to Provincial guidelines (see BCCNM license).
  • Ability to read, write, and speak English proficiently and understand verbal and written instructions.
  • Computer skills, including the ability to navigate basic computer programs such as Word, Excel, Outlook.
  • Demonstrates collaborative and team-working skills.
  • Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required to perform the duties of the position.
