In this role, you will have the opportunity to make a positive impact in the lives of others by providing high-quality patient care in the comfort of their own homes.

Key Responsibilities

  • Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
  • Complete initial and ongoing assessments to identify the needs of home health patients and develop personalized care plans.
  • Administer medication as prescribed by the patient's physician and educate patients and their families on proper home health care strategies and procedures.
  • Provide effective communication to patients/clients, their family members, team members, and other healthcare professionals to ensure seamless care coordination.

Requirements

  • LPN/RN license and current registration with the relevant regulatory body.
  • First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate.
  • Police Criminal Record Check with Vulnerable Sector Screening.
  • Community experience is an asset.
