Job Summary
We are seeking a skilled LPN or RN to join our team as a Home Health Care Professional. As a member of our team, you will have the opportunity to make a positive impact in the lives of our patients, providing high-quality care and support in the comfort of their own homes.
Responsibilities
- Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
- Complete initial and ongoing assessments to identify the needs of home health patients and develop personalized care plans.
- Administer medication as prescribed by the patient's physician and educate patients and their families on proper home health care strategies and procedures.
- Provide effective communication to patients/clients, their family members, team members, and other health care professionals.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and patient outcomes.
Requirements
- LPN/RN license in good standing.
- First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate.
- Police Criminal Record Check with Vulnerable Sector Screening.
- Community experience is an asset.
