About the Role

The Licensed Practical Nurse (LPN) is a key member of our care team at Retirement Concepts, responsible for coordinating care delivery and providing individualized resident-centered care. This role reports to the Director of Care and is responsible for ensuring the highest quality of care for our residents.

Responsibilities:

  • Assess residents' needs and implement individualized care plans to meet their unique requirements.
  • Liaise with physicians, community agencies, families, and other disciplines to ensure seamless care delivery.
  • Administer prescribed medications and treatments within the scope of practice.
  • Monitor and report changes in residents' physical and psychological conditions, documenting changes in their health records.
  • Participate in Interdisciplinary Care Conferences and care plan reviews to ensure current and individualized care plans.
  • Support administrative requirements, including admissions, discharges, and maintaining confidentiality.
  • Provide coaching and direction to care staff, using in-the-moment education to improve resident care.
  • Maintain up-to-date knowledge of community policies and procedures, including emergency procedures and safe work practices.

Qualifications and Skills:

  • Graduate of an approved Practical Nursing program and current licensure with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in long-term care an asset.
  • Ability to read, write, and speak English proficiently and understand verbal and written instructions.
  • Computer skills, including basic navigation of programs such as Word, Excel, and Outlook.
  • Demonstrates collaborative and team working skills.
  • Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required.
