We are seeking a highly motivated and dedicated Registered Nurse to join our team in Primary Care at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities in Primary Care settings.
- Develop and implement care plans in collaboration with the interprofessional team, taking into account the patient's primary care plan and personal goals of care.
- Conduct initial and ongoing client assessments, using clinical and diagnostic reasoning to inform nursing care and interventions.
- Communicate effectively with patients, families, and healthcare teams, using verbal, written, and computer communication means.
- Participate in team huddles and other activities to promote communication, cooperation, and collaboration across disciplines, ensuring continuity of care.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings, including experience developing and providing formal education to patients, families, or the general public.
- Current valid driver's license.
- Experience working in a team environment, with a focus on person-centered care and population health.
- Knowledge of adult learning principles and facilitation skills to effectively educate patients and others about topics essential to health care and well-being.
- Ability to operate related equipment, including relevant computer applications.
