Nurse Practitioner

We are seeking a dedicated and compassionate Registered Nurse to join our Primary Care team in Fort St. James. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the well-being of our community.
Job Summary:The successful candidate will be responsible for providing clinical follow-up care to patients, developing and implementing care plans, and collaborating with the interprofessional team to ensure seamless care delivery. You will also have the opportunity to work in a dynamic and supportive environment, with a focus on person-centered care and community engagement.
Responsibilities:- Provide clinical follow-up care to patients, including assessments, interventions, and education
- Develop and implement care plans in collaboration with the interprofessional team
- Collaborate with community health professionals to ensure comprehensive care delivery
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Contribute to the development and implementation of policies and procedures related to primary care
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
- Current valid driver's license
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
