Disability Support Specialist

We are seeking a highly skilled and dedicated Disability Support Specialist to join our team at the Government of Prince Edward Island. As a key member of our AccessAbility Supports program, you will play a vital role in delivering disability specific supports and services to eligible individuals with disabilities.
Job DescriptionThe Disability Support Specialist will be responsible for managing a caseload of clients, providing direction and case management support to deliver a range of disability specific supports and services. This includes working collaboratively with clients, their families, and support networks to develop personalized services and support plans that focus on maximizing client opportunities for full citizenship.
Key responsibilities include:
- Providing guidance, direction, and support to clients and their families utilizing a collaborative case management approach;
- Facilitating the development of personalized services and support plans to individuals with disabilities and their families;
- Identifying, leading, coordinating, and collaborating with the broader community, health, and education systems to ensure continuity of support plans for clients across the continuum;
- Analyzing and interpreting results from complex functional assessments on activities of daily living, which forms the basis of the client support plan;
- Accurately assessing clients' situations, taking into consideration environmental, individual, familial, and social perspectives;
- Proactively assessing and planning, as well as being responsive to changing abilities and needs of clients and families;
- Processing initial and ongoing applications for services and supports eligible under the program based on needs, within defined parameters;
- Assessing eligibility for financial supports to address expenses related to basic living, assured income for clients, in accordance with program guidelines;
- Responding and assessing emergency situations presented by applicants/clients related to areas such as housing, medical transportation, food, etc., to promote independence and self-sufficiency by empowering clients;
- Communicating effectively with clients and/or families regarding eligibility, support plans, navigation to other service providers or the broader social and health system or community on clients' needs related to their disabilities;
- Liaising with health professionals and hospital staff for clients who will be hospitalized for extended periods to determine potential in-hospital and transitional supports;
- Providing and promoting educational and marketing to individuals, public, community, and service organizations about supports and services available to persons with disabilities;
Salary: $70,000 - $80,000 per annum
