Job Summary:

The Government of Prince Edward Island seeks an experienced and compassionate AccessAbility Supports Coordinator to join our team. As a key member of our department, you will play a vital role in delivering disability-specific supports and services to eligible individuals.

The ideal candidate will possess strong case management skills, excellent communication abilities, and a passion for promoting inclusivity and accessibility.

Key Responsibilities:

• Collaborate with individuals, families, and support networks to develop personalized service plans.

• Provide guidance, direction, and support to individuals with disabilities and their families.

• Analyze and interpret results from functional assessments to inform support plans.

• Identify and lead efforts to coordinate community, health, and education systems to ensure continuity of support plans.

• Process applications for services and supports, assessing eligibility and ensuring compliance with program guidelines.

• Communicate effectively with clients and families regarding eligibility, support plans, and navigation to other service providers.

Requirements:

• Strong understanding of disability support needs and case management principles.

• Excellent analytical, communication, and interpersonal skills.

• Ability to work collaboratively with diverse stakeholders, including individuals, families, and community organizations.
