About the Opportunity

We are seeking a highly skilled and experienced Frailty Coordinator to join our team at Nova Scotia Health Authority. The successful candidate will provide leadership and support in the development, implementation, evaluation, and ongoing support for Early Mobility Programs and other Frailty Network initiatives.

The Frailty Coordinator will work closely with the Program Manager and other stakeholders to identify, manage, and prevent frailty within our organization. This will involve leading inpatient EMP staff throughout the zone, educating on the use of the Clinical Frailty Scale (CFS), and collaborating with interdisciplinary healthcare teams to implement and evaluate this work.

The ideal candidate will have a Bachelor's Degree in a regulated healthcare discipline, such as Occupational Therapy, Physiotherapy, or Nursing, and at least three years of relevant experience in healthcare delivery, quality improvement, program development, design, implementation, and evaluation.

The successful candidate will also have excellent research, data collection, report writing, facilitating, time management, organizational, and problem-solving skills. They will be able to work independently and collaboratively to successfully implement quality initiatives in a busy healthcare setting.

We are looking for a candidate who is passionate about improving care for older adults based on current best practices and published evidence. The successful candidate will be able to work in a fast-paced environment and prioritize multiple tasks and responsibilities.

Key Responsibilities:

  • Develop and implement Early Mobility Programs and other Frailty Network initiatives
  • Lead inpatient EMP staff throughout the zone
  • Education on the use of the Clinical Frailty Scale (CFS)
  • Collaborate with interdisciplinary healthcare teams to implement and evaluate this work
  • Provide education related to care of hospitalized older adults to frontline staff and clinicians
  • Oversee the coordination, training, implementation, and ongoing development of the mobility program members

Requirements:

  • Bachelor's Degree in a regulated healthcare discipline
  • At least three years of relevant experience in healthcare delivery, quality improvement, program development, design, implementation, and evaluation
  • Excellent research, data collection, report writing, facilitating, time management, organizational, and problem-solving skills
  • Ability to work independently and collaboratively to successfully implement quality initiatives in a busy healthcare setting
  • Passion for improving care for older adults based on current best practices and published evidence
