About the Role

The Geriatric Mobility Program Coordinator plays a pivotal role in leading and supporting the establishment, execution, assessment, and ongoing enhancement of Early Mobility Programs and various Frailty Network initiatives as designated by the Nova Scotia Health Authority. This position is integral in fostering a deeper understanding of Frailty through effective identification, management, and prevention strategies across the organization, all while aligning with the Mission, Vision, and Values of the health authority.

Key Responsibilities

  • Provide leadership to inpatient Early Mobility Program staff throughout the region.
  • Deliver educational sessions on the Clinical Frailty Scale (CFS) to interdisciplinary healthcare teams.
  • Oversee the coordination, training, and ongoing development of mobility program members.
  • Facilitate education related to the care of hospitalized older adults for frontline staff and clinicians.
  • Engage in clinical interventions, including care plan development and patient mobilization, in accordance with professional licensing.

Qualifications

We invite candidates who possess the following qualifications:

  • A Bachelor's Degree in a regulated health care discipline such as Occupational Therapy, Physiotherapy, Nursing, or another regulated health profession.
  • Current license/registration with the relevant regulatory body.
  • A minimum of three years of pertinent experience in healthcare delivery, quality improvement, program development, and evaluation.
  • Proficiency in health records and electronic medical records (EMR).
  • Demonstrated ability to work both independently and collaboratively to implement quality initiatives in a dynamic healthcare environment.
  • Strong computer skills, particularly in Microsoft Office Suite, with a focus on PowerPoint, word processing, spreadsheets, and data management.
  • Exceptional skills in research, data collection, report writing, facilitation, time management, organization, and problem-solving.
  • A solid understanding of Frailty and familiarity with the Clinical Frailty Scale (CFS).
  • A commitment to enhancing care for older adults based on current best practices and evidence.
  • Flexibility to work weekends as required.
  • Competencies in additional languages are considered an asset, with a preference for French.

Work Schedule

  • Permanent, Full-time position; 75 hours bi-weekly.
  • Flexible work hours, including weekdays and weekends.

Compensation and Benefits

The hourly compensation for this role is competitive, and successful candidates may qualify for a comprehensive benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
