Job Summary:

The Canadian Red Cross is seeking a Transitional Support Services Worker to support clients during the transition from hospital to their home. The successful candidate will work with the Program Manager to ensure a smooth transition and provide support to clients in accordance with established service plans.

Key Responsibilities:

  • Provide support to clients during the hospital discharge process and transition to their home.
  • Complete basic errands and home management duties as required, including preparing nutritious meals and maintaining a safe and comfortable environment.
  • Conduct risk assessments of clients' homes using standardized tools.
  • Facilitate client and caregiver engagement, referrals to community support services, and discharge processes.
  • Identify and report any unsafe equipment or situations, and handle and store hazardous products safely.
  • Provide companionship to clients within therapeutic boundaries.
  • Observe and report any concerns related to clients or their environment to the Program Coordinator.
  • Maintain confidentiality of clients, caregivers, and the Canadian Red Cross.

Requirements:

  • Standard First Aid and CPR-C certification.
  • Fluency in English is required, French is an asset.
  • Previous experience working with elderly clients is an asset.
  • Interpersonal skills, including dependability, punctuality, and ability to manage stress.
  • Ability to follow care plans as identified by the appropriate funder agency.
  • Self-starter with analytical and problem-solving skills, and ability to work in an environment with ambiguity.

Working Conditions:

  • Duties are primarily conducted in clients' residences.
  • Hours of operation are from 8:30 am to 4:30 pm, Monday to Friday.
  • Irregular hours may be required, but never outside of regular hours.
