About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.

Key Responsibilities
  • Patient Assessment and Care Planning: Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans.
  • Service Coordination: Link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.
  • Resource Management: Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected.
Requirements
  • Professional Qualifications: Membership, in good standing, with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
  • Experience: 2+ years of recent experience in community health or a related field.
  • Skills: Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills, as well as effective time management, prioritization, and organizational skills.
  • Additional Requirements: A valid driver's license and access to a reliable vehicle, proficiency in a Windows environment, and the ability to accurately complete required documentation, reports, and forms.
Preferred Qualifications
  • Experience with Diverse Patient Groups: Experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
  • Case Management Experience: Case management experience or recent related community experience.
  • Language Skills: Ability to speak French or another second language.
