Provide Essential Support During Client Transitions

The Canadian Red Cross is seeking a dedicated Home Care Support Specialist to join our team. In this vital role, you will play a crucial part in supporting clients as they transition from hospital back home. Your compassionate care and expertise will ensure their safety and well-being during this important time.

Your Responsibilities:

  • Assist clients with the transition process, adhering to established service plans.
  • Perform essential tasks such as meal preparation, light housekeeping, and maintaining a safe home environment.
  • Conduct thorough risk assessments and address any potential safety concerns promptly.
  • Facilitate meaningful interactions between clients, caregivers, and community resources.
  • Offer companionship within therapeutic boundaries and communicate any concerns to the Program Coordinator.

What We Look For:

  • Standard First Aid and CPR-C certification is required.
  • Fluency in English is essential; French proficiency is an asset.
  • Prior experience working with elderly clients is highly valued.
  • Strong interpersonal skills and the ability to effectively solve problems are crucial.
  • You must be able to work both independently and collaboratively as part of a team.

Work Environment:

This role primarily involves providing care in clients' homes. Standard working hours are Monday to Friday from 8:30 am to 4:30 pm, with occasional flexibility required within these operating hours.
