Job Summary

We are seeking an experienced Assessor Coordinator to join our team at Saskatoon City Hospital.

About the Role

The Assessor Coordinator will provide case management services, including assessment, planning, monitoring and evaluation. They will be responsible for single entry access and ongoing coordination for community-based services within the SHA.

Key Responsibilities
  • Assess client/patient needs and develop individualized care plans
  • Coordinate with healthcare professionals to ensure seamless delivery of care
  • Provide education and support to clients/patients and their families
Requirements
  • Bachelor of Science in Nursing (BScN) or equivalent degree
  • Registered/eligible for registration with the appropriate licensing body
  • Knowledge of SHA, community resources, and applicable programming
Working Conditions

Travel may be required to provide service to sites within the network. Preference is given to HSAS members as per the SAHO/HSAS Collective Agreement.

Salary and Benefits

This full-time temporary position offers a competitive salary range of $36,721 to $44,902 per year, commensurate with experience and qualifications. Additional benefits include opportunities for professional growth and development.
