Job Description

This role will involve working as part of a multidisciplinary team to provide primary care services to residents of the Northwest Territories. The successful candidate will be responsible for assessing patients, developing care plans, and implementing interventions to improve health outcomes. They will also contribute to health promotion and disease prevention efforts through education and community outreach.

Key Responsibilities:
  • Provide comprehensive primary care services to patients, including assessment, diagnosis, treatment, and follow-up
  • Develop and implement individualized care plans that address the physical, emotional, and social needs of patients
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
  • Participate in health promotion and disease prevention initiatives, including education and community outreach programs
Requirements

To be considered for this role, candidates must have:

  • A Master's degree in Nursing and certification as a Nurse Practitioner or an MSc-Nurse Practitioner
  • At least 2 years of relevant experience in primary care or a related field
  • Demonstrated expertise in assessment, diagnosis, treatment, and follow-up care
  • Excellent communication and interpersonal skills
