Job Description

Humber River Health is seeking a skilled Registered Practical Nurse to join our dynamic team on the 9 East Nephrology Unit in the Acute Inpatient Medicine Program.

About the Role
  • Provide comprehensive nursing care to patients with complex nephrology needs, utilizing your expertise and technical skills to deliver high-quality patient outcomes.
  • Collaborate with interdisciplinary teams to coordinate care, develop individualized plans of care, and facilitate smooth transitions between departments.
  • Utilize computerized systems for accurate documentation and data collection, ensuring timely access to information and facilitating informed decision-making.
Requirements
  • Bachelor's degree in Nursing or equivalent experience as a Registered Practical Nurse (RPN) in a related field.
  • Current RPN registration with the College of Nurses of Ontario and current Basic Cardiac Life Support (BCLS) certification.
  • One year recent medical experience in a hospital setting, preferably in nephrology, with a focus on patient care, education, and advocacy.
  • Experience with Peritoneal dialysis, Continuous Ambulatory peritoneal dialysis, and continuous cycling peritoneal dialysis required. Completion of Nephrology 1 and 2 courses or enrollment in progress.
  • Excellent organizational skills, prioritization abilities, and attention to detail, ensuring seamless delivery of care and efficient use of resources.
  • Demonstrated ability to effectively communicate with patients, families, and healthcare professionals, promoting collaboration and transparency throughout the care continuum.
  • Able to work effectively in a fast-paced environment, navigating multiple priorities and competing demands while maintaining a commitment to excellence and patient-centered care.
