Social Worker, Social Services Coordinator

About the Role:
The CIUSSS du Centre-Ouest-de-l'Île-de-Montréal is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in improving the social functioning of individuals, families, groups, or communities, addressing their psychosocial and community needs.
Key Responsibilities:
- Receive and analyze social and psychosocial service requests, providing guidance and referrals as needed.
- Intervene in crisis situations, offering support and short-term interventions.
- Collaborate with interdisciplinary teams to support clients, families, and caregivers.
- Evaluate stress factors and living conditions, including housing, income, and access to services.
- Assess the capacity of family and social networks to support or compensate for individual limitations.
- Detect and identify psychosocial situations that may impact treatment or an individual's well-being.
Requirements:
- Bachelor's degree in Social Work or related field, or equivalent qualifications recognized by the Ordre des travailleurs sociaux et thérapeutes conjugaux et familiaux du Québec (OTSTCFQ).
- Membership in OTSTCFQ.
- At least one year of experience as a Social Worker.
- Functional proficiency in English.
