About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Royal Columbian Hospital in New Westminster, BC.

As a Patient Care Coordinator, you will work collaboratively with a multidisciplinary team to provide high-quality patient care. Your primary responsibility will be to assess, plan, implement, and evaluate nursing care for patients, ensuring their physical, emotional, and spiritual needs are met.

Key Responsibilities

  • Assess patient/resident/family's actual and potential strengths and limitations, including physiological, psychological, socio-cultural, and spiritual needs
  • Provide a supportive environment for patient/resident/family, promoting involvement in health care decisions and respecting personal choices
  • Contribute to the development and ongoing changes to the individualized plan of care, attending and providing input into multidisciplinary care planning meetings and family conferences
  • Perform admissions, discharges, and transfers of patients/residents according to agency operating policies and procedures
  • Document and update all relevant care and procedure information to ensure patient/resident safety and comfort

Requirements

To be successful in this role, you must have:

  • Graduate of a recognized practical nursing program or equivalent combination of education, training, and experience
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
  • Ability to communicate effectively, both verbally and in writing
  • Ability to deal with others effectively
  • Ability to organize work and implement a plan of care
  • Knowledge and competencies to work at full scope of practice as currently required by the BCCNM, including administering medications
