Job Summary

The Community Support Counselor – Addiction Specialist is a key member of the Capacity Building Team at St. Joseph's Healthcare Hamilton. This role involves providing expertise in consultation for patients with concurrent disorders and addictions across in-patient and out-patient programs within all medical and mental health and addictions programs.

Key Responsibilities

  • Provide support to the interdisciplinary team in utilizing evidence-based treatment for addictions/concurrent disorders
  • Demonstrate advanced clinical reasoning skills
  • Work in partnership with the Concurrent Disorder outpatient program and the RAAM Clinic
  • Participate in the development and implementation of treatment plans for patients with complex addiction/medical issues
  • Provide clinical support to patients transitioning from hospital to community
  • Collaborate with multidisciplinary teams to ensure seamless integration of psychiatric and substance abuse interventions

Requirements

  • Bachelor's Degree in Psychology, Sociology, or relevant discipline
  • Evidence of ongoing advanced addictions education from an accredited educational institution
  • Registration with the college of psychotherapy (preferred)
  • Two years recent related experience working in a mental health setting utilizing an evidence-informed concurrent disorder framework
  • One year specific experience in an addiction treatment or medical setting (preferred)
  • Current First Aid/CPR certification
  • CPI-Crisis Prevention Training (or successful completion as part of clinical orientation)
  • Demonstrated knowledge of relevant legislation, including the Mental Health Act, Healthcare Consent Act, and Substitute Decisions Act
  • Demonstrated knowledge of clinical psychiatry and psychiatric medications
  • Demonstrated knowledge of and ability to access formal and informal community services and resources
  • Demonstrated competence in evidence-based addiction individual and group-based counseling techniques and interventions
  • Demonstrated ability to develop and maintain effective professional and therapeutic relationships with patients
  • Demonstrated skill in the identification and assessment of withdrawal symptoms and the application of appropriate interventions
  • Demonstrated ability to provide clinical support to patients with complex addiction/medical issues transitioning from hospital to community
  • Demonstrated crisis intervention, case management skills, and knowledge of behavioral techniques
  • Excellent communication, interpersonal, and problem-solving skills
  • Demonstrated ability to function effectively within a multidisciplinary team and ability to work independently
  • Demonstrated computer skills, including use of Microsoft Office
  • Excellent organizational and time management skills, including ability to prioritize workload
  • Good work performance and attendance record
