Social Worker I MSW In-Patient Psychiatry

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health Authority. As a Social Worker, you will play a vital role in providing comprehensive social work and psychosocial assessments to patients suffering from acute episodes of mental illness and substance misuse/dependence.
Key Responsibilities- Conduct comprehensive social work and psycho-social assessments of patients and families, using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Develop and coordinate care plans and community services for patients requiring facility placement and/or community support, collaborating with hospital and community services from a social work perspective.
- Communicate and collaborate with mental health and related community service agencies and family physicians, making referrals to appropriate resources when longer-term treatment or follow-up is required.
- Act as a resource to patients and their families, providing information and support regarding community resources and access to these resources.
- Participate in the family support program, providing educational and support services, and reviewing and evaluating the program to provide feedback on potential improvements.
- Master's Degree of Social Work from an accredited school of Social Work, plus one year's recent, related experience in a mental health treatment setting.
- Current full registration with the BC College of Social Workers.
- Current Class 5 Driver's License and access to a personal vehicle for business-related purposes, as required.
