About the Role

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health Authority. As a Social Worker, you will play a vital role in providing high-quality care and support to individuals and families navigating mental health and addiction issues.

Key Responsibilities
  • Conduct comprehensive social work and psychosocial assessments of patients suffering acute episodes of mental illness and substance misuse/dependence.
  • Develop and coordinate care plans and community services for patients requiring facility placement and/or community support.
  • Communicate and collaborate with mental health and related community service agencies and family physicians.
  • Act as a resource to patients and their families by providing information and support regarding community resources and access to these resources.
  • Provide comprehensive therapeutic clinical counselling services to patients suffering acute episodes of mental illness and families.
  • Participate as a member of the mental health program and local committees and planning bodies.
Requirements
  • Master's Degree of Social Work from an accredited school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • Current Class 5 Driver's License and access to personal vehicle for business-related purposes.
