Geriatric Psychiatry Social Worker

We are seeking a skilled Social Worker to join our Geriatric Psychiatry Outreach team. As a key member of our multidisciplinary team, you will provide assessment, treatment, consultation, and follow-up services to mentally ill elderly clients. Your expertise will help us deliver high-quality care and support to our clients and their families.
Responsibilities- Conduct comprehensive psycho-social assessments and develop treatment plans in collaboration with other team members.
- Provide counseling and crisis intervention to clients and families through home visits and outreach.
- Offer consultation, support, and education to families and caregivers of mentally ill elderly clients.
- Collaborate with healthcare professionals and community agencies to ensure seamless care coordination.
- Develop and maintain relationships with community resources and services to support clients and families.
We are looking for a candidate with a Master's degree in Social Work and at least three years of experience working with geriatric psychiatry clients. You must be registered with the BC College of Social Workers and possess a valid Class 5 BC Driver's license. Strong communication and interpersonal skills are essential for this role.
