  • Key Responsibilities:
  • Engage in meaningful dialogue with patients to assess their needs and challenges, considering physical, emotional, psychological, and social factors.
  • Develop tailored care plans to address clients' specific needs and goals, using a client-centered approach.
  • Establish measurable goals to provide clarity, track progress, and ensure targeted interventions and services.
  • Collaborate with a multidisciplinary team to ensure a holistic and coordinated approach to patient care.
  • Offer ongoing support to patients, providing guidance and encouragement to help them navigate challenges and achieve their goals.
  • Assist patients with identifying, referring, and navigating community resources to help them reach their target goals.
  • Conduct regular follow-up visits to assess patient progress, addressing emerging issues and adjusting care plans as needed.
  • Qualifications:
  • Completion of a Bachelor of Social Work from an accredited university (Master of Social Work an asset)
  • Registration with the Ontario College of Social Workers & Social Service Workers
  • Minimum 5 years of experience working as a Social Worker
  • Experience working in a community setting is an asset
  • Valid Ontario driver's license and a reliable vehicle
  • Proficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans
  • Familiarity with community resources and programs throughout the Toronto Central geography
  • Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting)
