We are seeking a dedicated Community Support Worker to join our team at Improving Lives In-Home Care. As a Community Support Worker, you will be responsible for providing support and assistance to clients in their homes, promoting independence and well-being.

Key Responsibilities:
  • Prepare reports and case histories to document client progress and needs
  • Develop and implement individualized service plans to meet client goals and objectives
  • Assist clients with daily living activities, such as household chores, meal preparation, and personal care
  • Provide companionship and emotional support to clients
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
Requirements:
  • Secondary (high) school graduation certificate
  • 1 year to less than 2 years of experience in a related field
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
Work Setting:

Group home
