Job Title: Community Support Worker

Job Summary: We are seeking a compassionate and dedicated Community Support Worker to join our team at Improving Lives In-Home Care. As a Community Support Worker, you will provide essential support and care to individuals in their own homes, promoting independence and well-being.

Key Responsibilities:

  • Prepare reports and case histories to document client progress and needs
  • Develop and implement individualized care plans to meet client goals and objectives
  • Assist clients with daily living activities, such as bathing, dressing, and meal preparation
  • Provide companionship and emotional support to clients
  • Work collaboratively with clients, families, and healthcare professionals to ensure comprehensive care

Requirements:

  • Secondary (high) school graduation certificate
  • 1 year to less than 2 years of experience in a related field
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills

Work Setting: Group home

Work Conditions and Physical Capabilities: Physically demanding, ability to work under pressure

Support for Mature Workers: Provides staff with awareness training to create a welcoming work environment for mature workers

Support for Visible Minorities: Provides diversity and cross-cultural training to create a welcoming work environment for members of visible minorities

Work Term: Permanent

Hours: 35 to 40 hours per week
