Job Summary

We are seeking a compassionate and dedicated Community and Social Services Worker to join our team. As a Community and Social Services Worker, you will be responsible for providing support and care to individuals in their homes, promoting independence and well-being.

Key Responsibilities
  • Prepare reports and case histories to document client progress and needs
  • Develop and implement individualized care plans to meet client goals and objectives
  • Assist clients with daily living activities, such as bathing, dressing, and meal preparation
  • Provide emotional support and companionship to clients
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Participate in training and professional development to enhance skills and knowledge
Requirements
  • Secondary (high) school graduation certificate
  • 1 year to less than 2 years of experience in a related field
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • Physical stamina to perform tasks that require standing, walking, and lifting
