Palliative Care Nurse

About the Role
Spectrum Health Care is seeking a dedicated and compassionate Palliative Care Nurse to join our team. As a Palliative Care Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals and their families in the Etobicoke community.
Key Responsibilities
- Provide holistic, patient-centered care that includes symptom management, coordination of care, patient, family, and caregiver education, emotional support, and advocacy for assigned patients and/or caseload of patients.
- Collaborate with the interdisciplinary team to create personalized care plans based on the patient's unique needs and goals of care.
- Participate in clinical huddles and case conferences with team members such as physicians and allied health.
- Practice in accordance with the College of Nurses of Ontario (CNO), the Regulated Health Professions Act, and Spectrum Health Care standards, policies, and procedures.
- Travel within assigned region between patients' residences or facilities.
Requirements
- Currently registered and in good standing with the College of Nurses of Ontario.
- Minimum of two-years nursing experience.
- Must have experience working with palliative care patients.
- Successful completion of any of the following is considered an asset: Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or Fundamentals of Palliative Care The Comprehensive Advanced Palliative Care Education (CAPCE).
- Current police check suitable for working with vulnerable sector.
- Valid Ontario driver's license and reliable vehicle with willingness to travel.
- Demonstrates problem-solving and critical thinking skills.
- Proficiency in English both written and verbal.
- Ability to work independently within the scope of the role and prioritize workload. Strong time management skills are required.
- Possesses the physical ability to perform the day-to-day duties described in the duties and responsibilities.
- Demonstrates proficiency in the use of technology.
- Excellent patient assessment and teaching skills.
- Ability to triage client needs and escalate concerns appropriately.
- Basic proficiency in Word, Excel, and ability to learn software.
