Job Summary:


Spectrum Health Care is seeking a skilled Registered Nurse to join our team of healthcare professionals in providing exceptional care to patients in need of palliative care. As a Palliative Care Nurse, you will be responsible for delivering patient-focused, safe, and holistic nursing care, working collaboratively with other members of our specialized palliative care team.



Key Responsibilities:



  • Provide patient-centered care, ensuring that nursing care provided to patients is based on current and best practices to achieve successful outcomes
  • Continually assess, anticipate, identify, and respond to new and changing needs of patients
  • Travel between patients' homes within a specific geographical region


Requirements:



  • Minimum of two years' experience in nursing, with community nursing experience preferred
  • Currently registered and in good standing with the College of Nurses of Ontario (CNO)
  • Must have experience working with palliative care patients
  • Proficiency in English, both verbal and written required
  • Valid Ontario driver's license and reliable vehicle with willingness to travel
  • Excellent patient assessment and teaching skills
  • Problem-solving and critical thinking skills
  • Physical ability to perform daily duties
