Job Summary

We are seeking an experienced Crisis Intervention Specialist to join our team in the Critical Care department. This role will provide a full range of social work services to patients and their families admitted to CVICU/B5ICU/CICU (for non-trauma/non-cardiac service patients) within the Trauma program.

Responsibilities

The successful candidate will be responsible for:

  • Providing psycho-social assessments and counseling to patients and families;
  • Supporting patients and families through end-of-life care, advance care planning, and goals of care conversations;
  • Coordinating transition planning and crisis intervention;
  • Liaising with community resources and healthcare teams;
  • Supporting patients and families through the trauma psychosocial assessment process.
Requirements

To be successful in this role, you will require:

  • A Master of Social Work degree;
  • Membership in the Ontario College of Social Workers and Social Service Workers in good standing;
  • A minimum of two years of experience working in a hospital setting, preferably in critical care;
  • Exceptional communication skills, including negotiation and conflict resolution;
  • Demonstrated knowledge of complex care management and health system access;
  • Ability to balance collaborative practice with self-direction and manage multiple tasks in a fast-paced environment.
Compensation and Benefits

This full-time position offers a competitive salary range of $93,104 - $123,164 per annum, commensurate with experience and qualifications. Additional benefits include a comprehensive pension plan, generous vacation time, and opportunities for professional development.
