We are seeking a skilled and compassionate Nurse Practitioner to join our team at PHSA. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional healthcare services.
Key Responsibilities- Provide direct patient care services, including assessments, diagnoses, and treatment plans, in a trauma-informed and patient-centered approach.
- Collaborate with healthcare professionals to develop and implement comprehensive care plans, ensuring seamless transitions and continuity of care.
- Conduct health education and promotion activities to empower patients and families to take an active role in their healthcare.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and organizational standards.
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years' recent experience providing trauma-informed acute or community health nursing care/services to patients with complex physical, psychiatric, and/or substance abuse/misuse disorders.
- Successful completion of Occupational First Aid II (OFA II) or OFA III techniques.
- Working knowledge of community health nursing theory, including mental health and addictions, communicable diseases, primary care, and practice within a patient/family-centered approach.
- Knowledge of the BC Mental Health Act and other relevant facilities and community resources.
- Working knowledge of facilitation, mediation, and conflict resolution techniques, with the ability to adapt to new or unexpected events.
- Awareness of and commitment to learning and understanding the Truth and Reconciliation Commission's 94 Calls to Action, In Plain Sight Report, and other related reports.
