Job Summary
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Holy Family Home in Winnipeg. As an LPN, you will provide high-quality nursing care to our residents, working collaboratively with our healthcare team to ensure the best possible outcomes.
Key Responsibilities
- Provide professional nursing services to designated residents, including independent nursing services and administration of prescribed medical treatments.
- Collaborate and interact with the healthcare team to coordinate resident care and achieve optimal outcomes.
- Participate in the development and implementation of individualized care plans, ensuring that the unique needs and preferences of each resident are met.
- Conduct regular assessments and evaluations to identify changes in resident condition and adjust care plans accordingly.
- Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
Requirements
- Graduate of an approved Licensed Practical Nursing Education program.
- Active registration with the College of Licensed Practical Nurses of Manitoba (CLPNM).
- BLS certification.
- Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and healthcare team members.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Physical and mental health to fulfill the responsibilities of the position.
